Based on multiple authoritative sources, a typical binder should contain sections for driver qualification, medical and drug records, vehicle‑related documents, operating authority and insurance evidence, fuel and tax permits, hazardous‑materials paperwork and other supporting materials.
1. Driver qualification and safety records
  • Completed driver application – part of the driver qualification file (DQF).

  • Copy of current CDL. and three‑year driving history (Motor Vehicle Record).; update annually..

  • Previous‑employer , showing past driving experience.

  • Medical examiner’s certificate (DOT medical card) – valid medical certification and renewal proof.; drivers must also carry their Med Card..
